Who do you have questions or worries about? Is it yourself, someone in your family, or a friend?
What are you looking for - is it information, advice, support or help to cut down or stop doing cigarettes, alcohol or drugs?
Are there specific questions you want us to answer (you might want to write these down), or do you just need to talk something over?
Do you want us to contact anyone else on your behalf?
